Before I start, I was totally uninvolved with the entire engagement. It occurred entirely while I was at work, and thus, I can not comment on it directly. As such, I'm not even really qualified to give a fair assessment of how things occurred, yet I feel that it is a really interesting anyway. My observations are simply my own, and almost entirely founded upon the after-action reports.

1) This was a well-planned kill, not an off-the-cuff thing. This kill is a demonstration of what's capable when you plan and organize properly, not a demonstration of how my grandmother could kill a Titan blindfold - Evil Thug comments that this is the third time Thulsa's safe had been found, but only the first time a proper attack force was ready.
Correct; when you field a titan, expect a lot of people, your enemies specifically, to be investing huge amounts of isk and time in taking it down. This was the case here.

2) The safe was found at the time of logout, not the time of login.
This is my understanding, but I was not involved.

3) Thulsa was present at the safe for several minutes after login, on the mistaken assumption that the Titan was safe while cloaked. Seleene admits not being au fait with the current speed of dedicated probing ships...
Also correct, as far as I am aware. Also, remember that a titan is not a small thing... decloaking it when you know it's general location is somewhat easier than a buzzard .

4) It's possible to tank a Ragnarok's Doomsday. I'd have guessed it was possible to tank a Sabre against an Avatar given the high base armor resists, and I suppose an explosive-tanked shield-tanking interdictor makes sense too. If anyone wants to try and theorycraft setups to tank Thermal/Kinetic Doomsdays, that'd be interesting too.
I suppose this would have been a bit surprising to someone who hadn't considered this option. Some unconventional fits with EFT might yield interesting results in this. I was surprised.
The one thing I've noted through all of this is the fact that people seem to be vying for change with every super-cap killed. This is a good thing, in my opinion, as we learn the faults of each ship, and can propose changes. There are so few super-caps in the game, that it takes everyone of them to be looking for issues to find them all [in comparison to normal ships].
The issue at hand here, however [as I see it at least] is that people feel that it was unjust as to what occurred. Mind you, I can hardly call myself unbiased in this matter, as I clearly am, yet I'd like to emphasize that this wasn't a freak accident by any means. There was no accidental Doomsdaying, nor logging off, nor evil tactics at hand. There was a lot of hard work, by a lot of people on both sides. I am under the firm belief that the game mechanics work exactly as they should.
Further more, and infact, back on topic, I'd like to raise a fairly simple question: Speaking simply in terms of logic, with ship and probe sensors being as sophisticated as they are in EVE, would you not expect them to be able to detect the presence of a ship the size of a small planetoid? This is simply for your head and not at all to do with game balance or otherwise.
